Anticoagulation

The clinical pharmacist monitors the patients INRs, make appropriate dosing adjustments and schedules the next appointment. The pharmacist ensures appropriate understanding of the risk involved and the benefits of anticoagulation. They will assess for any possible adverse effects, dietary concerns and educate patient on monitoring parameters.

Respiratory

The clinical pharmacist monitors the patients response to inhaler regiment, make appropriate dosing adjustments and schedules the next appointment. They ensure appropriate utilization of inhalers. They will assess for possible adverse effects and adherence.

Endocrinology

The clinical pharmacist monitors the patients response to treatment regimen including ording and assessing lab values. They will make appropriate dosing adjustments and schedule the next appointment. The patient will be assessed for possible adverse effects, dietary education and home monitoring as needed.
